Promotion Terms & Conditions

PurposeMed Inc., doing business as Frida (collectively, “PurposeMed”) promotion (“Promotion”) begins on March 11, 2024, at 12:00 a.m. Pacific Time (PT) and ends on April 11, 2024, provided it’s not terminated earlier by PurposeMed (the “Promotion Period”). Participation in the Promotion constitutes your full and unconditional acceptance of, and agreement to be legally bound by, these Promotion rules (the “Promotion Rules”). 

1. ELIGIBILITY:
   To be eligible for the Promotion, a person must, in each case during the Contest Period: (a) be a resident of Canada (excluding Quebec), (b) have reached the age of majority in their province or territory of residence, and (c) have signed up to complete an ADHD diagnosis assessment (“Assessment”) with the link provided by Frida, and (d) have paid for and completed the Assessment.
   
   

2. HOW TO ENTER:

   Assessments that are incomplete, irregular, have been submitted through incorrect means, or do not conform to or satisfy any condition of these Promotion Rules shall be disqualified. You acknowledge and accept the decisions of PurposeMed, or those acting on behalf of PurposeMed, as final and binding, without right of appeal, in all respects, including, without limitation, as to eligibility and/or disqualification of the eligibility of the Assessments.
   
   Limit: One entry per person. Attempts to receive additional Rebates by using multiple identities or addresses will disqualify you from this and future promotions.
   
   

3. REBATE:

   a. Rebate: Eligible patients who book, pay for, and complete an Assessment with PurposeMed during the Promotion Period are entitled to a C$50 rebate (“Rebate”). PurposeMed will contact you via email within two weeks of completing the Assessment. PurposeMed will inform you of the details of the Rebate and deliver the same by approximately 14 days following your provision of the necessary information for delivery of the Rebate. 
   

   b. If a patient: (a) declines the Rebate; or (b) fails to provide PurposeMed with any information required to provide the Rebate to the eligible winner in a timely manner, then they will be disqualified (and will forfeit all rights to the Rebate). Rebates must be accepted as provided and are not transferable or refundable. No substitutions except at PurposeMed’s option.

   
   

4. GENERAL CONDITIONS:

   PurposeMed reserves the right, at its sole discretion, to disqualify any individual (and all of his or her entries) who tampers with the entry process.
   

   a. This Promotion is subject to all applicable federal, provincial and municipal laws and the Promotion is void where prohibited by law.
   

   b. PurposeMed reserves the right to cancel, suspend, or amend this Promotion in any way, without prior notice or obligation.
   

   c. PurposeMed accepts no responsibility for loss, damage or claims caused by or resulting from the, Assessment, Promotion or the Rebate.
   
   d. PurposeMed will not be liable in the event of an error, technical problem, computer virus, bugs, tampering, unauthorized intervention, fraud, technical failure or any other cause beyond the reasonable control of PurposeMed that interferes with the proper conduct of this Promotion as contemplated by these Promotion Rules.
   

   e. PurposeMed reserves the right, in its sole and absolute discretion, to require proof of identity and/or eligibility (in a form acceptable to PurposeMed – including, without limitation, government issued photo identification): (i) for the purposes of verifying an individual’s eligibility to participate in this Promotion; (ii) for the purposes of verifying the eligibility and/or legitimacy of any Entry entered (or purportedly entered) for the purposes of this Promotion; and/or (iii) for any other reason PurposeMed deems necessary, in its sole and absolute discretion, for the purposes of administering this Promotion per these Promotion Rules. Failure to provide such proof to the satisfaction of PurposeMed in a timely manner may result in disqualification in the sole and absolute discretion of PurposeMed. The sole determinant of the time for the purposes of a valid Entry in this Promotion will be the Promotion server machine(s).
   
   f. PurposeMed respects your right to privacy. By entering this Promotion, each entrant expressly consents to PurposeMed, its agents and/or representatives, storing, sharing and using the personal information submitted with his/her entry only for the purpose of administering the Promotion and in accordance with PurposeMed’s Privacy Policy (available at: https://www.PurposeMed.com/privacy-policy) unless the entrant otherwise agrees to receive further communications from PurposeMed.
