We want to make ADHD care accessible for all Canadians. Whether you have a referral as a practitioner or clinician, or would like to self-refer to Frida, please choose one of our below referral options:

  1. Option 1: For Patients

    Visit our online referral form HERE to self-refer.

    Referrals will be directed to our free 5 minute assessment to understand if Frida is a fit for your needs. You will be prompted to book your first diagnostic assessment with one of our clinicians after the questionnaire.

  2. Option 2: For Practitioners and Clinics

    Please download our fillable PDF referral form HERE and fax to our Care team at 855-719-0483 once completed.

    A member of our Care team will contact the patient within 48 business hours.
